I got this in Windows 7, notice how the window contents are shifted to the left and upwards. At the time the window was in resolution 1000x800 on 16bit on Win7 x64:

this behavoir was explained several times.
you must use video modes multiples of 16x8 (width x height), if you use other modes you need use dump_type=1 and restore_type=1.
